Home Health
Christian Care & Fellowship Square communities partner with Covenant Home Health to provide supportive home health services as well as Medicare-certified outpatient therapy, physical therapy, and skilled home health services.
Our caring staff provides supportive services and skilled home health services in the comfort of your home. In addition to Medicare, we also accept a variety of insurance plans.
Supportive services are private-pay and do not require a physician's order. In addition to the services listed below, a social worker is available to assist patients in locating community resources.
Skilled Home Healthcare
To qualify for Skilled Home Healthcare, patients must have a physician's order and a medical necessity for the skilled services of a nurse or therapist. Patients must also meet Medicare's definition of home-bound status*.
*Medicare defines home-bound status as "trouble leaving your home without help (like using a cane, wheelchair, walker, or crutches; special transportation; or help from another person) because of an illness or injury."
Outpatient Therapy
After a procedure, you may need outpatient therapy to get back to normal. To make it easier, we are pleased to provide door-to-door transportation via golf cart at no extra charge (campus availability may vary).
If you don't already have doctor's orders, our staff will coordinate with your doctor to obtain them for you. We will also verify insurance benefits - your job is to focus on improving your strength and well-being!

Short-term Rehab & Skilled Nursing
A New Chapter Begins: Our Transition to Short-Term Rehab for Seniors.
Christian Care Health Center was established in 1979 and was the first level of care offered on our Phoenix campus. And now, a new chapter begins as we shift our focus from long-term care to short-term rehab for seniors.
As part of this transition, we are excited to announce that the entire building will be renovated to provide a welcoming atmosphere and a renewed sense of community. You can read more about our transformation here.
