This ensures you can focus on healing comfortably at home, just one of the many benefits of a senior living community that offers a full continuum of care.
Be sure to inform your case manager of your preference so we can work together to facilitate your return to Christian Care when you are ready to begin your rehabilitation journey.
Upon admission, our Discharge Planning Team will develop a personalized care plan for your needs. Throughout your stay, you will be informed about your discharge plan and the team’s recommendations upon discharge.
